CCC Campus Receives Bronze Award
Community Care Center has started working on the AHCA/NCAL Natiional Quality Award this year, wihich is a progressive three-step process, to encourage the continuous learning and development of intergrated quality systems to achieve performance excellence. Each progressive step requires a more detailed demonstration of quallity integration and performance.
The Bronze recognizes that the provider has developed a foundation to begin a journey of continuous quality improvement.

Dietary Services
Each day begins with the most important meal of the day, breakfast which will be provided at their leisure beginning at 6:30 a.m. Our lunch is served at 11:30 and supper at 5:30.Not only do our residents have several selections on the menu to choose from but often enjoy special cookouts and holiday party meals We feel that dining at any time should be a very social and relaxing time and we feel we have created just such an environment and special area. Our dining room area has an excellent view of the porch patio area adjoining the dining room. Residents have the opportunity, as they wish, to enjoy this area at anytime. It is especially nice for coffee gatherings during the morning hours in Spring and Summer. The patio is decorated with seasonal plants, tables and comfortable seating.

Programs and Activities
Exercise and focus on maintaining the functional abilities of our residents is of great importance to us as well as each resident and their family. We offer various types of programs of which many are specialized to the needs of each individual resident. Our spacious rehab room offers the availability of many more choices for types of automated/mechanical exercise equipment use and can accommodate a large class group at one time. A daily group exercise class is held 7 days a week at 10:30 in our large multipurpose room adjacent to the dining area.Our rehab staff offers many years of experience and knowledge benefiting our residents greatly.

Social Services
A Licensed Social Worker is on duty, addressing resident and family needs, five days a week. Counseling, discharge planning, assistance with community services, and group therapy are just a few of the services offered by our Social Worker at Community Care Center.

Enriched Environment
It is our goal to enrich the lives of our residents and keep them active here as well as in the community. We aspire to give our residents the opportunities to do the things they may never had a chance to do or to continue to do the activities they have always enjoyed doing. We are constantly asking them for ideas and suggestions because it is important for them to do the things they want to do not us. They are our number one priority and that's the way it should be. Our large multipurpose room offers plenty of room for family visits and small family gatherings, facility parties and potlucks, watchng tv or a movie and all of our other daily activities. If our residents have shared their life with a special pet we hope to fill that void. We offer enjoyment and the companionship of our facility dogs, Bonnie, Clyde and Buck. Each resident's room is ready to accommodate easy television cable connection and phone installation. Arrangements for cable hookup and monthly billing can be made with Community Care Center and phone connection can be arraged through our local area carrier, Qwest. We have a transport bus and it has been a blessing as it enables us to take the residents to places such as Amana Colonies, Pella Tulip Festival, Iowa Cubs games, Danish Windmill Inn and so much more.

Nursing and Medical Services
The most important asset at Community Care Center is the outstanding, competent, reputable and qualified nursing staff which we provide at all times. Our utmost professional nursing staff will provide ongoing professional and quality care for your loved one and are always available to visit with you about any care giving concerns you might have.

Our facility Doctor or associate is always on call twenty four hours a day if the need arises. All residents have the choice of retaining their own doctor to serve them or may request the services of the facility doctor at any time.

Professional Services
Our facility is located next to the Stuart Professional Plaza which provides easy access for services often needed, from other professionals such as: medical, (MD. & PA.), Dentistry, Optical and pharmacy if you chose to use them. Other in-house services we offer, if chosen to receive, are timely visits from our PT/OT, and podiatrist and Hospice Care. A Hospice room has been added furnished to provide a more private and homelike environment for patients and their families. Our facility offers skilled care services not only to our "in-house" patients but also to our community citizens. Our seperate out patient therapy room offers privacy to those receiving special skilled treatment. Other services include a weekly visiting beautician ready to serve our residents providing wash, sets and styling as well as permanents for the ladies per family/resident request.

The Willows Assisted Living Complex
The Willows is designed to offers the opportunity for those 55 and older to enjoy luxury living in the privacy of their own apartment. Included amenities and service make this life style more secure and enjoyable. The Commons area provide a homelike atmosphere as tenants are served their meals family style.The Willows offers two apartment floor plan options to select from.
